Our network of 3,800 primary care doctors and specialists provide care for some 300,000 members who have health insurance coverage through HMO, Medicare Advantage or Managed Medi-Cal plans. We are actively shaping the future of healthcare. We participate in four Accountable Care Organization (ACO) style arrangements for major employers including CalPERS, the City and County of San Francisco and the University of California (San Francisco), as well as a multi-employer commercial ACO for many of our members in San Joaquin County. These successful collaborations involve various health plans, Dignity Health hospitals (formerly CHW) as well as UCSF, one of the nation’s most respected academic medical centers. In each, our case management and care management platforms are extended to hospitals, community-based physicians and academic physicians in order to create a more seamless healthcare experience for the patient. Whether through ACOs, patient centered medical homes or narrow networks, Hill Physicians is working to provide high-quality, coordinated care to our members.
